Job summary

Responsibilities

• Assess patient clinical and psychosocial needs and develop a care plan.
• Coordinate with family to implement and evaluate the plan of care.
• Collaborate with interdisciplinary team for care coordination.
• Facilitate safe transitions to next level of care.
• Document per RHS standards and regulatory requirements.
• Delegate and prioritize tasks to ensure quality patient care.
• Respond to emergency situations.
• Evaluate resources to meet patient needs.
