• Anyone who worked as Per Diem at a facility in this program in the last 6 months cannot be submitted.
• Anyone who worked as a traveler at a facility in this program in the last 6 months cannot be submitted.
• Anyone currently working at a facility in this program cannot be submitted.

Special Requests:Must be separated from BILH for 6 months for travel, permanent, or per diem roles.

1 Charge/Lead/Supervisor reference required from within the last 12 months
