Job summary

Knox Community Hospital's Cardiac Rehab department seeks an RN Navigator/Chest Pain Coordinator to guide patients through the care pathway and coordinate services.

You'll implement and evaluate plans of care, collaborate with the healthcare team, and ensure timely documentation while maintaining safety and age-appropriate care. This full-time role requires an Ohio nursing license and BLS certification, with BSN preferred.

Qualifications

• Graduate of an accredited nursing program; BSN preferred.
• Current Ohio RN license.
• AHA BLS/CPR certification required.
• Cardiology and heart failure experience preferred.

Responsibilities

• Guide patients through coordination of care in the cardiac rehab pathway.
• Develop, implement, and evaluate plans of care for chest pain/cardiac rehab patients.
• Collaborate with the healthcare team and document timely.
• Adhere to organizational policies and regulatory requirements.
