Responsabilidades

• Assist individuals in managing chronic conditions and accessing healthcare services.

Job Description

Community Health Worker (CHW) is a pivotal link between healthcare providers, social services, and the community working out of the Emergency Department. CHWs are instrumental in ensuring that vital health resources and services are accessible to those who need them most. The role is designed for individuals who are deeply committed to promoting wellness and improving health outcomes.

Essential Duties

• Assist individuals in managing chronic conditions and accessing healthcare services.
• Arranges chronic disease education and referrals as needed for patients and families.
• Identifies issues, which may impact medical outcomes based on comprehensive assessments that may include the patient’s medical condition, support systems, finances, living situation, housing, behavior cognition, function/abilities, and the patient’s choice and/or preference.
• Problem solves with patients and their family members to maximize their ability to follow preventative, remedial and/or rehabilitative recommendations.
• Conduct outreach to patients and community members, to provide education, assessment, social support, and resources.
• Implement individual and community assessments to determine health related needs and/or identify risks
• Facilitate patient access to appropriate medical and specialty providers well as other care coordination team support specialists
• Provides care coordination, case management, and system navigation to improve patient outcomes, reduce readmissions and cost and enhance the patient/family experience and assure access to care.
• Provide culturally appropriate health education and information.
• Support patients and community members in navigating the healthcare system.
• Support patients and community members in circumnavigating the required applications for financial support or insurance coverage.
• Advocate for individual and community health needs.
• Collect data and relay information to healthcare providers to inform care across the continuum.
• Facilitate access to social services and other community resources.
• Collaborate with healthcare providers and community organizations with health screenings/clinics etc.
• Build trusting relationships with community members.
• Empower individuals to adopt healthy behaviors and lifestyle changes.

Job Requirements

• General Requirements Obtain Basic Life Support (BLS) certification issued by the American Heart Association within 45 days of hire and maintain
• Community Health Worker certification is preferred
• Work Experience See education for required experience.
• Experience working with underserved populations with medical and social needs in a social service and/or community-based setting is preferred
• Education Associate’s Degree in social work, health/human services, community/public health or related field and 3 yrs related experience (social work, health education, community outreach, community health or human services)
• Education Bachelor’s Degree in social work, health/human services, community/public health or related field and 1 yr related experience (social work, health education, community outreach, community health or human services)
• Education Bachelor’s in Social Work/BSW prepared is preferred
• Specialized Knowledge and Skills Knowledge and use of community resources; social, health and public agencies and services.
• Experience in data collection and reporting
• Strong organizational and problem-solving skills
• Ability to work independently and as part of a team
• Basic computer skills
• Cultural competency and sensitivity
• Preferred training in motivational interviewing
