Job summary

Tempus Unlimited is seeking a community-based Skills Trainer for our Personal Care Attendant Program serving the Cape Cod, Nantucket and Martha's Vineyard area in Massachusetts. This full-time role involves educating consumers and surrogates on managing their PCA program and completing department-wide tasks.

The position requires travel within a 60-mile radius of the home address, a valid driver's license, reliable vehicle, 8:30 am–4:30 pm schedule, and bilingual English preferred.

Qualifications

• Experience providing services for people with disabilities or in PCA programs preferred.
• Strong communication, organizational, and writing skills required.
• Basic computer skills in Microsoft Office/Outlook and database navigation.

Responsibilities

• Follow up on assigned functional skills training within contract timelines.
• Assess consumer/surrogate ability to manage PCA services.
• Train consumers/surrogates to manage PCA program per Mass Health and PCM contract.
• Maintain confidentiality in all consumer-related issues.
• Attend trainings/meetings; conduct weekly 1-1 meetings with supervisor.
• Document progress notes and required documentation within deadlines.
• Complete 10-12 in-person home visits weekly; travel within service area.

Job description

A community-based Skills Trainer for our Personal Care Attendant Program. The Skills Trainer will educate consumers and surrogates if applicable on how to successfully manage their consumer-directed hands-on care program. The Skills Trainer will also be primarily responsible for other skills training department-wide tasks and processes. The Personal Care Management Skills Trainer will cover the Cape Cod, Nantucket and Martha's Vineyard in Massachusetts area including up to a 60-mile radius from your home address.

• $21 an hour
• Mileage reimbursement
• Bilingual English preferred in all languages
• Full-time position, 35 hours a week
• Monday - Friday, 8:30 am to 4:30pm
• Travel required, cover a 60-mile radius of your home address
• Must have a valid driver's license
• Must have reliable vehicle to travel throughout assigned service area
• 2-day orientation in Stoughton in person
• Training in Stoughton for 2-3 weeks

Essential Job Functions

• Follow-up on assigned functional skills training in a timely manner as determined by contract.
• Assess consumers and/or surrogate’s ability to manage PCA services.
• Train consumer and/or surrogate with the skills needed to manage their PCA program as outlined in the Mass Health Service Agreement and according to the Personal Care Management (PCM) Contract.
• Maintain confidentiality in all consumer related issues.
• Attend in-person or virtual trainings and meetings as requested by Supervisor or Manager, including monthly 1-1 meetings with your supervisor.
• Demonstrate a working knowledge of program policies and procedures and Mass Health regulations.
• Demonstrate a commitment to the Independent Living philosophy of consumer control.
• Must report all suspected incidents of consumer sexual/physical abuse and neglect to the Disabled Person Protection Commission (DPPC) within 24 business hours.
• Work within company policies and in accordance with the PCM Contract in order to complete and track for receipt of required documentation within required timelines.
  • Assigned referrals and intake documentation
  • Service Agreements and Assessments for re-evaluations
  • Supportive documentation for intakes and re-evaluations as needed
  • Return all phone calls from surrogates and consumers within 72 business hours
• Work within policies to complete and submit internal requirements:
  • Document administrative and communication progress notes in real time.
  • Document visit and travel progress notes within 48 business hours.
  • Complete 10-12 in person home visits per week.
  • Complete and submit skills training paperwork within 48 business hours.
  • Verify and approve Dayforce timecard daily.
  • Update Outlook calendar daily.
  • Provide a weekly productivity report as requested by immediate supervisor.
  • Use TripLog when traveling to conduct any in person home visit or any business-related travel including office.
  • Submit mileage and expense forms by the 15th of each month.
  • Return all required email communications within 48 business hours.
• Follow up with Supervisor and/or any other department on requests for skills training and tracking reports:
  • Non-billing, overbilling, overtime and ineligible consumers.
  • Provide training and ongoing support for EVV.
  • Provide Issue Focus training as requested.
  • Consumer status updates.
  • Pending Intake, Quarterly, Surrogate Change and Annual Reviews.
  • Perform other duties as assigned by the Skills Trainer Supervisor or Management.

Qualifications

• Experience providing services for people with disabilities and knowledge of PCA programs is preferred.
• Experience or education in teaching or counseling is helpful.
• Knowledge of community resources and social service systems is beneficial.
• Excellent communication, organizational, and writing skills.
• Basic computer skills in Microsoft Office/Outlook and the ability to navigate databases.
• Specialized training provided as needed.

Required Education

• Bachelor’s Degree preferred; GED or High School Diploma required.
