Job summary

Qualifications

• Master’s degree in Social Work is required.
• National Certification in Case Management (CCM) is preferred.
• At least 1 year of medical social work/case management experience.
• Strong English reading/writing and interpersonal skills.

Responsibilities

• Conduct assessments and care planning for CAP beneficiaries.
• Coordinate transitions and reduce readmissions with community supports.
• Perform interdisciplinary needs assessments monthly.
• Document via the eCAP system and coordinate with teams.
• Provide crisis intervention and linkage to services as needed.
