Job summary

Elite Care in Gresham, OR is seeking a CADC to provide 1:1 and group support for residents dealing with substance use and co-occurring disorders. You will work with a multidisciplinary team to apply motivational interviewing, harm reduction, and behavior support to foster change and safety.

The role involves developing individualized addiction support plans, coordinating with care teams, and delivering both individual and group education.

Qualifications

• Excellent interpersonal skills required.
• Ability to respond to emergencies and crisis behavior (may involve physical intervention).
• Maintain cooperative relationships with colleagues and residents.
• Proficient in English communication, both written and spoken.
• Knowledge of residents' needs and behavioral supports.
• Creative approaches to meet emotional and physical needs.
• Proficient with basic computer tasks (emails, spreadsheets).
• High integrity and ethical conduct.
• Ability to delegate while managing daily activities.
• Knowledge of state and federal regulations.

Responsibilities

• Screen referrals for admission and develop addiction support plans within 15 days.
• Align support plans with each resident's service plan and explain to residents.
• Participate in service planning and act as liaison to case managers.
• Provide individual coaching and group teaching to staff on strategies from the plans.
• Coordinate with Health Services on addiction education and interventions.
• Collaborate with Behavior Specialist to mitigate behaviors related to substance use.
• Monitor, reevaluate, and revise addiction support plans as needed.
