Job summary

Optum is a global organization delivering care aided by technology to improve health outcomes. This role focuses on psychosocial assessment, action planning, and coordination with members, families, and health care providers in the Long Beach area.

Pay ranges from $24.00 to $43.00 per hour, based on full-time employment, with a comprehensive benefits package and HIPAA-compliant practices. In-person work at local skilled nursing facilities is required several days a week.

Qualifications

• Undergraduate degree or equivalent experience.
• BSW with 1+ year clinical healthcare experience or successful completion of relevant internship.

Responsibilities

• Establish a safe, effective care management plan using vetted resources.
• Conduct a comprehensive psychosocial assessment and develop a member-specific action plan.
• Collaborate with members, families, and healthcare providers.
• Identify and refer to financial, psychosocial, community, and state services.
• Advocate for members navigating the health system.
• Address age-specific communication needs with diverse populations.
• Identify high-risk members needing social services intervention.
• Maintain records of treatment plans and outcomes.
• Participate in interdisciplinary team meetings.
• Serve as a resource for colleagues and providers on complex cases.
• Understand insurance products, benefits, and governing regulations as they apply to the plan.
• Protect PHI per HIPAA standards.
• Display professionalism and positive attitude.
• Assess and respond to danger to self or others per licensure.
• Perform additional duties as assigned.
• In-person at 3-4 skilled nursing facilities in Long Beach area weekly.
• Four 10-hour shifts, 8:00–18:30 or five 8-hour shifts M-F.

Primary Responsibilities:

• Utilize vetted and approved resources to establish a safe, effective care management plan
• Complete a comprehensive psychosocial assessment for each member and develop a member-specific action plan
• Collaborate with members, families, and healthcare providers
• Identify and initiate referrals to financial, psychosocial, community, and state supportive services
• Advocate for members and assist them in navigating the healthcare system
• Address age-specific communication needs and engage effectively with members and families from diverse backgrounds
• Identify and intervene with high-risk members requiring social services
• Maintain records of social care treatment plans, interventions, and outcomes
• Participate in interdisciplinary team meetings
• Act as a resource to department teammates, organization teammates, and providers for complex members
• Understands insurance products, benefits, coverage limitations, insurance, and governing regulations as it applies to the health plan
• Use, protect, and disclose members' protected health information (PHI) only in accordance with Health Insurance Portability and Accountability Act (HIPAA) standards
• Portray a positive attitude and professionalism when interacting with others
• Trained to assess and respond to danger to self or others in alignment with state licensure standards
• Perform additional duties as assigned
• In Person at 3-4 skilled nursing facilities in the Long Beach area a week
• Four 10hr shifts, 8-6:30 or five 8hr shifts M-F

Required Qualifications:

• Undergraduate degree or equivalent experience
• BSW - 1+ years of clinical experience in a healthcare setting, or successful completion of a relevant internship
• Driver's License and access to reliable transportation

Preferred Qualification:

• Bachelor's degree from a CSWE-accredited School of Social Work or a bachelor's degree in a Behavioral Science-related field
