Qualifications

Responsibilities

• Performs a comprehensive assessment on all assigned patients.
• Communicates changes in patient's condition promptly.
• Documents outcomes and updates the care plan with measurable goals.
• Collaborates with patient, family and health care team for discharge planning and referrals.
