Job summary

PathCare Health in Brooklyn, NY is seeking a Family Nurse Practitioner (FNP) to provide comprehensive primary and preventive care to patients across the age spectrum in a diverse, dynamic setting.

You will diagnose, manage chronic conditions, prescribe medications, and order tests while collaborating with physicians, nurses, and other healthcare professionals to ensure holistic patient care and optimal outcomes.

Qualifications

Responsibilities

• Prescribe medications and order diagnostic tests as necessary
• Collaborate with healthcare teams, including physicians, nurses, and other healthcare professionals, to ensure seamless and effective patient care
• Stay up-to-date with the latest medical research, guidelines, and technologies to continuously improve patient outcomes
• Participate in quality improvement initiatives and patient safety programs to maintain the highest standards of care
