Job summary

A community service organization in Blacksburg, Virginia is seeking a Case Manager to provide vital case management services for individuals with mental illness and substance use disorders. This role requires collaboration with the Jail Diversion Team to ensure clients receive appropriate services and support. Candidates must have at least a High School Diploma and experience in social services. Additional compensation will be considered for multilingual candidates. Competitive starting hourly rate is $18.75 depending on experience.

Qualifications

• High School Diploma is required with at least one year of experience in a setting working with individuals with substance use disorders and/or possible co-occurring mental illness.
• Bachelor's Degree in Education, Social Work, Psychology, or Human Services related field is preferred.
• Registration as a QMHP or trainee is preferred.
• CSAC is preferred.

Responsibilities

• Provide case management services for individuals with mental illness and substance use.
• Coordinate services for clients enrolled in the Jail Diversion Program.
• Ensure compliance with program requirements and expectations.
• Attend treatment team meetings and maintain contact with referral sources.
