Job summary

Qualifications

• Medical Doctor - board licensed and board eligible to practice in the provision state.
• Doctor of Osteopathy - licensed and board eligible to practice in service state.
• Federal DEA license (state and federal) required for practice.

Responsibilities

• Assesses, diagnoses, and treats patients according to medical staff approved protocol and acceptable standards of practice.
• Provides medical services and health education to patients and family members.
• Performs necessary diagnostic and therapeutic procedures.
• Interprets information gathered during the diagnostic process and formulates appropriate plan of care.
• Provides quality care for patient, demonstrates appropriate health, safety, and aseptic practices.
• Maintains patient records and accurate information.
• Meets regularly with the care team for consultation and collaboration to promote quality care for patients.

Education

Medical Doctor - Board of Medicine Licensed and qualified to practice medicine in the state that services are provided and board certified or board eligible

Doctor of Osteopathy - Board of Medicine Licensed and qualified to practice medicine in the state that services are provided and board certified or board eligible and board certified or board eligible

Federal Drug Enforcement Admin - US Department of Justice Drug Enforcement Admin State and federal DEA license based on state of practice
