Job summary

New York Breast Health is seeking a Breast Nurse Navigator (RN) in Bohemia, NY. You will educate patients on biopsy prep, review pathology results, and guide follow-up care with radiologists and clinicians.

The role requires NY RN licensure, strong assessment and communication skills, and bilingual abilities. EMR documentation and care coordination are essential, with a full-time schedule and competitive hourly pay.

Qualifications

• Graduate from an accredited School of Nursing with NY license.
• Nursing experience with clinical competence preferred.
• Excellent assessment, communication, and organizational skills.
• Bilingual English/Spanish a plus.

Responsibilities

• Provide education on biopsy prep, post-procedure expectations, and follow-up by phone.
• Review and document pathology results and communicate with radiologist.
• Assist patients with follow-up appointments and referrals in EMR.
• Outreach to patients needing additional diagnostic imaging or follow-up.
• Maintain accurate and timely EMR documentation.

Notify patients of all benign pathology results and ensure understanding of recommendations. Assist patients in navigating follow-appointments and referrals and ensure all necessary orders areplaced correctly in the EMR. Conducting outreach to patients requiring additional diagnostic imaging or follow-up aftermammography screenings. Communicate with and assist patients who are due or overdue for annual mammography screenings. Maintain accurate and timely documentation in the EMR.We require:Graduate from an accredited School of Nursing and hold an unencumbered New York license.
