Christus Health is seeking a Breast Care Patient Navigator to support timely patient care and follow-up. You will advocate for patients navigating the breast care pathway, coordinates appointments, and joins an interdisciplinary team to ensure quality care and program success.

The role supports educational outreach and data collection to track outcomes, with multiple shifts available and a part-time work type. Training in BNCP is preferred but not required.

Qualifikationen

• Must have 1+ year breast imaging experience.
• Graduate of an accredited Radiologic Technology program.
• Basic computer experience required.

• Collects data, tracks outcomes, and supports strategic work plans for the Breast Care Team.
• Participates in educational outreach to physicians, office staff, diagnostic staff, and other key customers to maintain relationships.
• Performs all other duties as assigned.

Description

Summary:

Breast Care Patient Navigator serves as an essential part of the breast care team by supporting timely patient care and follow-up. This job is an advocate for the patient in their dealings with the healthcare community, helping patients navigate their way to timely and quality care and assisting the breast care team in patient care initiatives and programs.

Responsibilities:

• Meets expectations of the applicable OneCHRISTUS Competencies: Leader of Self, Leader of Others, or Leader of Leaders.
• Helps patients navigate their way to timely and quality care from suspicious findings to follow-up testing, while addressing tangible barriers to patient care.
• Facilitates appointments for consults and support services within established standards.
• Functions as a member of an interdisciplinary team to assess, secure, and coordinate appropriate services.
• Helps qualified patients secure services from any available grants from state and or national programs.
• Works with the Breast Care Team and others to collect data, track outcomes, and support strategic work plans.
• Participates in educational outreach efforts to the community and to key customers (physicians, office staff, diagnostic staff, etc.) to establish and maintain positive working relationships.
• Performs all other duties as assigned.

Job Requirements:

Education/Skills

• Graduate of an accredited program in Radiologic Technology or other accepted and approved equivalent required

Experience

• 1 year or more of breast imaging experience is required
• Basic computer experience required

Licenses, Registrations, or Certifications

• Mammography (M) by ARRT is required
• State Licensure required
  • Texas: MRT by TMB
  • Louisiana: LRT (R) or (F) by LSRTBE
  • New Mexico: RRT by MIRTP NMED
• Breast Navigator Certification Program (BNCP) preferred
