Job summary

Theoria Medical is seeking a part-time Advanced Practice Provider to serve local skilled nursing facilities. You will build lasting relationships with residents, provide high-quality care, and participate in a flexible schedule supported by intuitive technology.

Qualifications

• Active U.S. license required.
• Board certification accepted from ANCC, AANPCB, PNCB or AACN (nurse practitioner) or NCCPA (physician assistant).
• Eligible specialties include Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Geriatric Medicine.

Responsibilities

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounds and develop treatment plans.
• Prescribe medications and address acute/emergent issues.
• Communicate with facility staff via secure messaging or phone.

What You’ll Do

• Provide high-quality, patient-centered care to residents in skilled nursing facilities.
• Conduct daily rounding, prescribe medications/treatment plans, and address acute/emergent issues.
• Communicate with facility nursing staff via phone or secure messaging as needed.
• Maintain reliable on-site attendance for scheduled shifts.
• Ensure compliance with clinical standards, facility policies, and regulatory requirements.

Your Qualifications

Advanced Practice Providers must hold an active U.S. license and be board-certified by ANCC, AANPCB, PNCB, or AACN (for Nurse Practitioners), or by NCCPA (for Physician Assistants). Qualified specialties include, but are not limited to, Internal Medicine, Family Medicine, Emergency Medicine, General Practice, Physical Medicine and Rehabilitation, and Geriatric Medicine.
