Job summary

TriStar Centennial Medical Center in Nashville, TN is seeking a BMT Long Term Follow Up Coordinator to support post-transplant patients in the outpatient setting. You will collaborate with the transplant team to monitor physical, emotional, and financial needs and arrange follow-up visits and tests.

Qualifications

• Valid TN RN license.
• 3-5 years oncology experience including stem cell transplant and/or heme-malignancy, adult setting preferred
• Current BLS
• ONS Chemotherapy Certification preferred
• BMT CN required within 4 years of hire
• BSN or Master’s degree preferred but not required
• Apheresis experience, preferred but not required

Responsibilities

• Coordinate care of the post-transplant patient in the outpatient setting.
• Monitors patient/family after transplantation for physical, emotional, psychosocial, and financial needs and intervene when appropriate.
• Responsible for long-term follow-up, which includes but is not limited to: periodic updates on patient condition, coordinating follow-up visits with Blood and Marrow Transplant physician, scheduling disease restaging (and other tests as needed) services.
• Maintain awareness of outstanding lab/imaging results and relay to provider for prompt follow up.
• Coordinate referrals to physical therapy, home health, and other specialties as needed.
• Coordinate survivorship efforts between transplant team and outside physicians, optimizing patient recovery and health maintenance after transplant.
