Job summary

BJC Home Care is seeking a fully licensed Clinical Social Worker to provide psychosocial support to patients and families in Illinois Home Hospice. The role involves counseling, resource linkage, and discharge planning within a multidisciplinary team.

The position covers a 32-hour work week, Monday–Friday, with weekend on-call duties. Located in Glen Carbon, IL, the role requires strong communication and cultural sensitivity to diverse patient populations.

Qualifications

Additional Information About the Role

• Shift Information: Monday through Friday 8:00 Am – 4:30 PM (weekend on call 4 times a year) - 32 hous per week
• Home Office Location: - IL Hospice & Support Care - 2220 S State Hwy 157 - Ste 300 - Glen Carbon, Illinois 62034
• Coverage area: Will cover our entire service area for Illinois Home Hospice
• Case Load: 35-45
• Job Responsibilities: - Meeting with patients/families, determine resource needs and making referrals, determine social support needs, education and planning for end-of-life needs, counseling support needs for anticipatory grief. Meeting with patients and families to get consents signed for admission, covering end of life visits, filling in for other social workers as needed.
• Fully licensed Clinical Social Worker required

The Alton Intermittent Home Care Department of BJC Home Care Services provides home visits to patients in Alton, Illinois and several nearby counties, with 24 hour on-call home care nursing supervision. Role Purpose

Provides clinical social work services to address the psychosocial needs of patients and families. Provides individual and group counseling, linkage to community resources, expertise in cultural diversity and advocacy. Collaborates with the health care team and ensures timely, cost-effective discharge and transition planning.

Responsibilities

• Conducts screening and assessment activities to meet the needs of the discharged patient.
• Applies assessment findings to prepare a patient-centered plan of care.
• Applies and complies with all regulations and utilizes resources available to patients to manage their continuum of care.
