Job summary

AltaMed Health Services Corp. seeks a PrEP Navigator II to provide HIV/STI education, counseling, and navigation services for clients at high risk. You will refer clients to ACA enrollment, coordinate PrEP/PEP appointments, and assist with medication programs.

Requirements include a high school diploma, 2+ years in human services with HIV prevention or case management, and HIV test counseling certification (or obtain within 3 months). Bilingual English/Spanish preferred; must be 21+.

Qualifications

• Bachelor's Degree in social services or other health-related field preferred.
• Minimum of 2 years in a human services field with HIV prevention or case management experience required.
• Experience with Motivational Interviewing and client-centered harm reduction counseling is highly desirable.
• Certification as HIV Test Counselor or obtaining within 3 months of employment is required/ strongly preferred.
• Valid BLS or higher certification per AHA/Red Cross guidelines.
• Bilingual, English and Spanish, preferred; must be at least 21 years old.

Responsibilities

• Refer clients for ACA enrollment and navigate to PrEP/PEP services.
• Schedule medical appointments for PrEP/PEP and assist with Medication Assistance Programs.
• Recruit for PrEP, conduct client intake and assessments, and coordinate referrals.
• Monitor progress toward deliverables and represent AltaMed in internal/external groups.
• Mentor other team members and contribute to program development.

Job Overview

The PrEP Navigator II, Biomedical Program Infectious Diseases (ID) provides comprehensive HIV/STI Health Education, HIV counseling and testing, and facilitates HIV Prevention and PrEP/PEP Navigation Services to clients at high risk for infection or transmission of HIV. The Navigator is responsible for: 1)referring clients for ACA enrollment, 2)navigating clients onto PrEP/PEP services, 3)scheduling medical appointments for PrEP/PEP, and 4)assisting with Medication Assistance Programs. The Navigator conducts program recruitment for PrEP, client intake and assessment, facilitation/mentoring, and the coordination of referral services to programs as appropriate. The Navigator monitors and reports progress toward deliverables and represents AltaMed by serving on internal and external workgroups, collaborations, and coalitions. Lastly, the Navigator II supports mentoring of other team members.

Minimum Requirements

High School Diploma required. Bachelor's Degree in social services or other health-related field preferred. Minimum of 2 years of experience in a human services field, previous experience providing HIV prevention or case management services required. Minimum 2 years of experience in Motivational Interviewing and/or demonstrated capacity to deliver client-centered harm reduction counseling is highly desirable. Previous experience delivering case management services to low-income, at-risk populations is preferred. Certification: Existing certification as a HIV Test Counselor, Los Angeles Division of HIV and STD Programs, in advance of employment, or strongly Angeles Division of HIV and STD Programs in advance of employment or must obtain within 3 months of employment with AltaMed. A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines. Bilingual, fluent in English and Spanish, preferred, Must be at least 21 years of age, due to outreach requirements at local bars, clubs, and other venues relevant to program activities.

Compensation

$25.00 - $29.32 hourly
