LivWell Behavioral Health is seeking a Bilingual Youth Case Manager in Mesa, AZ. This full-time role involves guiding youth and families from intake through discharge, coordinating services, and providing compassionate, culturally informed support within an outpatient behavioral health setting.

The ideal candidate brings Spanish proficiency, strong organizational skills, and a commitment to collaborative care with children and families.

Qualifications

• Bachelor's degree in social work, psychology, or a related field preferred; relevant experience may substitute.
• Minimum of one year of experience (including internships or volunteer work) with children with mental health needs.

Responsibilities

• Intake, assessment & treatment planning from intake through discharge.
• Administer CALOCUS assessments and complete intake paperwork.
• Develop individualized service, safety, and treatment plans; update plans and documentation every six months.
• Coordinate supportive services and communicate progress with guardians weekly.
• Facilitate client advocacy and crisis navigation when needed.
• Lead Child and Family Team meetings and engage community resources.
