Health Connect America is seeking a licensed-eligible therapist to deliver high-quality mental health services. The role includes assessments, individualized treatment planning, and coordination with a multidisciplinary team across home, community, or office settings.

Ideal candidates hold a Master's in a related field, have at least two years’ experience with children or families, and are eligible for LPC/LMFT/LCSW licensure. We offer supportive benefits and growth opportunities.

Qualifications

• Master's degree in social work, counseling, or related mental health field is required.
• Two years' experience with children and/or families, including internships.

Responsabilités

• Provide high-quality therapeutic services to clients and deliver assessments.
• Develop and implement individualized treatment plans with goals.
• Document all client interactions and treatment plans timely.
• Collaborate with multidisciplinary team and participate in meetings.
• Engage in outreach to promote services and attract referrals.
• Maintain compliance with regulatory and organizational standards.
• Provide crisis intervention and safety planning as needed.
• Stay current with best practices and participate in training.
