Salary

• Master's Degree: $65,929 – $79,114
• Bachelor's Degree: $59,826 – $71,791

What You'll Do

• Work with individuals with developmental disabilities over the age of 3 years and their families, advocating for and securing necessary services.
• Participate in the assessment of individuals, their supports, and family needs.
• Develop, implement, and coordinate Individual Program Plans, completing required documentation.
• Manage a wide range of active cases, including creative planning and service acquisition.
• Conduct ongoing social assessments and overall case planning; collect data for interdisciplinary team use.
• Design, monitor, and revise program plans, analyze data, and identify barriers to progress.
• Initiate and terminate regional center funding and ensure purchased services are provided.
• Prepare required reports and documentation accurately and on time; maintain records in compliance with federal and state requirements.
• Advocate for individuals and families to ensure receipt of entitled services and encourage participation in community resources.
• Initiate referrals, monitor vendor service progress, evaluate availability and effectiveness, and identify gaps.
• Provide outreach services as assigned and participate in internal and community committees and task forces.

Requirements

Who You Are

• Knowledge of principles, objectives, and techniques of counseling individuals.
• Knowledge of normal and abnormal individual and group behavior, human growth and development, and family relationships.
• Knowledge of community structure and organization, and how to intervene constructively on clients' behalf.
• Ability to work independently, exercise judgment, and collaborate as part of an interdisciplinary team.
• Excellent communication skills, able to produce clear, concise, and professional documents.
• Ability to engage clients, families, and community agencies constructively.
• Ability to establish and maintain effective working relationships.

Qualifications

• Bachelor's Degree in social welfare or a related human services field; Master's Degree preferred.
• Fluency in ASL, including thorough knowledge of colloquial and technical terminology, and ability to speak and understand ASL as required.
• Experience in case management strongly desired.
• Successful candidates must have reliable transportation to visit programs on short notice.
