Job summary

CenterWell Senior Primary Care is seeking a Clinical Care Nurse (RN) to enhance patient outcomes. This clinic-based role involves supporting transitions of care, managing quality performance, and conducting patient outreach to provide chronic disease education.

The ideal candidate will have an active RN license, 3+ years of experience, and the ability to work collaboratively. The position is full-time, requiring a presence at assigned clinics in Dallas, Texas, Monday to Friday.

Qualifications

• 3+ years of clinical nursing experience in transitions of care.
• Active and unrestricted RN license required.
• Bilingual in English and Spanish is preferred.

Responsibilities

• Support safe transitions of care and improve patient outcomes.
• Conduct targeted patient outreach for care management.
• Collaborate with interdisciplinary teams for evidence-based interventions.
