Job summary

Trilogy, Inc. is seeking a Bilingual Recovery Counselor to assess and support clients in recovery from mental illness and co-occurring disorders.

The role involves delivering services in English and Spanish across home, community, and office settings and coordinating individual recovery plans aligned with client needs. The counselor will document care in the EMR, provide crisis on-call coverage, and link clients to community resources.

Qualifications

  • Bilingual in Spanish required; comfortable providing services in Spanish.
  • Must have one of the following requirements: High School Diploma or GED with two years of supervised clinical experience in a mental health setting, or an Associate’s or Bachelor's degree.
  • Valid Illinois driver’s license with daily access to a well-maintained vehicle and $100,000/$300,000 liability insurance.

Responsibilities

  • Assess clients’ personal, medical, emotional, social, and environmental situations through home, community, and office visits in English and Spanish.
  • Coordinate the establishment of an individual recovery plan with the client, their support system, and other care providers in English and Spanish.
  • Complete daily progress notes based on services provided to clients in a timely manner per organizational policies.
  • Document all client care in the Electronic Medical Record (EMR) and related forms (IMCANS, LOCUS, MAR, etc.).
  • Recognize and act on opportunities to move clients to appropriate levels of care; provide referrals to additional services.
  • Assist clients in identifying signs of de-compensation, assess for crisis situations, and provide stabilization support.
  • Provide psychoeducation, medication training, and monitoring according to Trilogy policy.
  • Coordinate with team nurse/pharmacy to ensure medication accuracy and MAR documentation.
  • Assist clients in improving independent living skills and daily tasks in English and Spanish.
  • Educate and assist with entitlements (SSI/SSDI, Medicare, Medicaid, LINK) in English and Spanish.
  • Understand representative payeeship and provide client money management services.
  • Transport clients to important appointments and support housing, budgeting, and social needs.
  • Advocate for clients and empower self-advocacy; engage in multidisciplinary team communication.
  • Participate in daily team meetings and remote/on-call coverage as assigned.

The Bilingual Recovery Counselor will assess the client’s personal, medical, emotional, social, and environmental situation to plan for treatment course through home, community, and office visits. They will provide individually-based motivational treatment and support to clients to assist them in their recovery from mental illness in English and Spanish. The Bilingual Recovery Counselor will be an advocate for clients, and link them to community services as needed. They will provide client-centered, strengths-based, and trauma-informed services to clients with severe mental illnesses and co-occurring substance abuse/addiction disorders utilizing a harm reduction approach in English and Spanish.

The Bilingual Recovery Counselor will provide at least 75% of all community support services in the community in English and Spanish. They will complete case management tasks with clients within a shared caseload. The Bilingual Recovery Counselor will be responsible for documenting all services provided to clients according to organizational policy and state requirements. The Bilingual Recovery Counselor will also provide crisis/on-call coverage as directed.

Responsibilities:
  • Assess client’s personal, medical, emotional, social, and environmental situation through home, community, and office visits in English and Spanish.
  • Coordinate the establishment of an individual recovery plan with the client, client’s support system, and other care providers in English and Spanish.
  • Complete daily progress notes based on services provided to clients in a timely manner per organizational policies and standards
  • Complete all necessary documentation related to client care, i.e. IMCANS, LOCUS, tracking forms in the Electronic Medical Record, etc.
  • Recognize and act on opportunities to move clients to appropriate levels of care; provide referrals to additional services as indicated.
  • Assist clients in identifying signs and symptoms of de-compensation, assess for crisis situations and or the need for stabilization through hospitalization. Work collaboratively with clients to identify coping skills to assist in managing symptoms and stressors experienced in English and Spanish.
  • Provide psychoeducation, medication training, and monitoring to clients in English and Spanish according to Trilogy policy
  • Coordinate with team nurse/pharmacy to ensure client medication accuracy
  • Documenting in real time on medication administration record (MAR)
  • Assist clients in learning and improving independent living skills; i.e. personal hygiene, housekeeping skills, nutrition, and shopping for food and personal items in English and Spanish
  • Educate and assist clients with applying for entitlements; i.e. Supplemental Security Income, Social Security Disability Insurance, Medicare, Medicaid, and LINK in English and Spanish
  • Understand the representative payeeship process per organization policy and provide individualized client money management services.
  • Accompany and transport clients to important appointments in the community and provide support with issues related to housing, substance use, budgeting, social support, and medication.
  • Advocate on client’s behalf and empower clients to advocate on behalf of themselves when appropriate
  • Communicate effectively within the team model for a multi-disciplinary approach to client care.
  • Actively participate in team communication via multiple electronic platforms
  • Engage regularly in daily team meetings to consult and collaborate on shared caseload
  • Provide on-call and crisis coverage as assigned.
  • Perform other related duties and/or projects as assigned
