Wheeler Clinic in Waterbury, CT seeks a Fee-for-Service Bilingual Outpatient Integrated Health Care Clinician to provide clinical and case management services across the lifespan in an integrated setting with primary care, behavioral health and dental services.

Qualifications include a Master’s degree in Marriage and Family Therapy, Counseling, Social Work or related license-eligible field or a doctoral degree in Psychology, with licensure eligibility as LPCA, LMFTA, LPC, LMFT or Psychologist.

Qualifications

• Master’s degree in Marriage and Family Therapy, Counseling, Social Work or related license-eligible field or a doctoral degree in Psychology is required.
• Must be licensed or license eligible LPCA, LMFTA, LPC, LMFT or Psychologist. If you are a MSW, a LMSW (permit or license) or LCSW will be required to begin employment.

Responsabilités

• Works as part of a multidisciplinary team to meet behavioral, primary, and dental health needs.
• Conducts program-specific screening and evaluation and formulates DSM-based diagnoses.
• Develops and documents clear clinical formulations and treatment plans with client/family input.
• Triages clinical emergencies including suicide and danger to others and applies evidence-based practices.
• Provides or refers to group therapy and documents clinical information timely.
• Makes clinically appropriate referrals for identified service needs in a timely manner.
• Communicates effectively with clients, families and providers; attends meetings and planning sessions.
• Meets direct service productivity goals and adheres to program goals and data reporting.

Formation

Master’s degree in Marriage and Family Therapy, Counseling, Social Work or related license eligible field or a doctoral degree in Psychology
