Suvida Healthcare is seeking a Nurse Practitioner for a dual role in direct patient care and provider performance. You will work with the Medical Director of Provider Performance on chart review, documentation improvement, and development of educational resources to elevate value-based care.

The position requires a Master’s in Nursing, NP licensure and board certification, and 3+ years in primary care. Bilingual English/Spanish is preferred; travel to other centers may be needed.

Formación

• Master’s degree or higher in Nursing is required.
• Current NP licensure and board certification are required.
• 3+ years of clinical experience as a Nurse Practitioner, with value-based care experience preferred.
• Experience in primary care and strong knowledge of clinical documentation/improvement.

Responsabilidades

• Provide direct patient care two days per week, including comprehensive assessment and management of acute and chronic conditions.
• Review clinical charts for completeness and documentation quality to support high-quality care.
• Identify gaps in clinical documentation to improve clarity and care planning.
• Develop and deliver provider education using chart reviews to improve clinical reasoning and documentation quality.
• Create, update, and maintain educational materials, clinical tools, and workflows.
