Job summary

Nemours is seeking an LPN for Primary Care at St. Francis Location. Bilingual English/Spanish applicants preferred. Hours are Monday through Friday, 8:30 AM to 5:00 PM or 9:00 AM to 5:30 PM.

The LPN will gather patient data, assist with procedures, administer medications and vaccines, perform point-of-care testing, and deliver developmentally appropriate care while supporting quality improvement efforts.

Qualifications

• Graduate of an accredited Licensed Practical Nurse program.
• Active DE LPN license is required.
• Current BLS certification from the American Heart Association is required.

Responsibilities

• Gather patient data and collaborate with the healthcare team on the plan of care.
• Safely administer medications and vaccines as prescribed by the provider.
• Assist the provider with procedures within the LPN scope of practice.
• Perform point-of-care testing.
• Provide age- and developmentally appropriate care to primary and specialty care patients.
• Document patient intake (height, weight, vital signs, head circumference, vision/hearing screens, reason for visit, medication reconciliation).
• Participate in departmental and system-wide quality improvement activities.
• Screen patient and family calls, gather information, escalate, and refer appropriately.
