You will collaborate with a multidisciplinary team to assess, diagnose, and design interventions for mental health and chronic conditions, coordinating care with community resources as needed. The role requires maintaining licensure and credentials, providing same-day and crisis services, and supporting clinic quality goals within PHC’s program.

Qualifications

• Master’s degree in Social Work.
• Current California Social Work license; credentialing requirements per PHC.

Responsibilities

• Functions as collaborative member of integrated care team.
• Readily accepts warm handoffs and provides follow-up recommendations to referring provider.
• Shares skills and interventions with team to facilitate coordinated treatment.
• Uses screening tools to assess mood, risk, and progress in treatment.
• Coordinates care with community resources as appropriate.
• Documents progress in medical records using objective methods.
