Job summary

Scan-Health-Plan is seeking a field-based RN to provide long-term care management for frail and culturally diverse older adults in Los Angeles County. You will assess, plan, educate, and coordinate services to prevent premature institutionalization while collaborating with a multidisciplinary team.

The role requires a CA RN license, BSN preferred, and 3+ years of home-visit experience. Travel 50%+ is expected; bilingual English/Korean is required.

Qualifications

• Nursing degree required; BSN preferred.
• Registered Nurse with active California license.
• 3+ years in health assessments, care planning, case documentation.
• Experience with older adults, home visits, and mental health/substance issues is desirable.
• Able to travel within LA County and work in a fast-paced environment.
• Bilingual in English and Korean is required.

We are building the future of care for older adults, grounded in purpose, accountability, and respect for the people and communities we serve.**This is a field/remote based role in LA County**The JobProvide long-term care management to frail, economically disadvantaged, culturally diverse older adults, disabled adults and caregivers in order to help them remain safely at home and to prevent premature institutionalization.You WillAssess new clients and determine appropriateness for the MSSP program, certify the client as nursing home eligible, conduct assessments and reassessments for identification of client’s medical, physical, and psychological needs.Ensure client’s needs are met through written care plan approved by the program supervisor and discussed verbally with the client and/or his representative.Provide ongoing health education to non-clinical staff members.Present cases at and participates in weekly team case conference per guidelines. Discuss during case conference complex health care issues for teaching purposes.Contact client’s physician as needed to discuss health problems identified during assessments.Provide education to the client or family member regarding topics including self-care, hygiene, and nutrition.Collaborate with staff, clients, physicians, home health, community services, family members, hospital discharge planners, and other professionals involved in the care of the client to assure appropriate services, and safe living environment below the cost of a skilled nursing facility.Maintain monthly documentation of clients along with other clinical documentation of relevant contacts with clients, service providers, health professionals, or other community agencies in a timely manner.Maintain accurate purchase of service records by completing accurate and timely monthly service authorizations and verifications to contracted vendors and following contract guidelines for service expenditures.Develop and implement care plans for assigned caseload.Help to maintain census and provide excellent customer service in the community by participating in ongoing outreach activities.Participate regularly in Quality Assurance chart reviews, completing other audits as required, helping to train new employees, participating in training sessions, and responding positively to suggestions for improvement.Increase knowledge by keeping current on Medicare and Medi-Cal policies, Federal and State legislation affecting health and social services, maintaining updated community resource information, attending trainings as scheduled, actively participating in supervision, and striving to improve professionally.Other duties as assigned.Your QualificationsNursing degree required. Bachelor's degree in Nursing (BSN) preferred.Registered Nurse required; Current and active California RN license in good standing.3+ years experience with health assessments, level of care criteria, care planning, and case documentation.Experience working with older adults and conducting home visits highly desirable, as is experience with mental health and/or substance abuse issues.Ability to travel within program’s catchment area and visits to client homes is required.Knowledge of environmental concerns of older adults. Theoretical understanding of the developmental needs for this population and means for intervention.Ability to perform home assessments, telephone monitoring, and travel to client homes throughout Los Angeles County.Ability to work as a part of a team, while at the same time performing job independently.Strong organizational, verbal, and written communication skills.Ability to work well with others required.Demonstrates keen attention to detail and customer focus.Ability to prioritize multiple and competing tasks.Ability to work well in a fast-paced and dynamic environment.Available to work Monday-Friday, 8am to 5pm with extended work hours, as needed and extensive simultaneous computer and telephone usage.Demonstrated customer focus and experience working in a case management setting, and a demonstrated knowledge of case management techniques is preferred.BILINGUAL- Must be bilingual in English/Korean.
