Job summary

Mindify is seeking an Internal Medicine Physician, Nurse Practitioner, or Physician Assistant to deliver primary care to adults at our Fair Lawn, NJ location. The role emphasizes preventive care, chronic disease management, and patient education, with coordination across specialists and behavioral health teams.

Qualifications

• Fluent English and Spanish required for patient communication and documentation.
• Active NJ license and board eligibility/certification in Internal Medicine for physicians, or NP/PA with primary care experience.
• Current DEA registration and prescriptive authority.

Responsibilities

• Conduct medical evaluations and diagnose adults.
• Manage acute and chronic conditions including hypertension, diabetes, and cardiovascular disease.
• Perform annual physicals, wellness visits, and preventive screenings.
• Develop and update individualized treatment plans.
• Order and interpret labs, imaging, and diagnostics.
• Communicate results and follow-up plans with patients.
• Prescribe medications and monitor effects.
• Educate patients on conditions, medications, and lifestyle.
• Document encounters accurately in the EMR.
• Coordinate care with specialists and behavioral health clinicians.
• Participate in meetings and quality improvement.
• Provide care in English and Spanish.
