Job summary

Qualifications

Responsibilities

• Provide in-home/community-based individual and family therapy to promote placement stability.
• Deliver a full range of clinical treatment duties for a caseload (individual, family, group).
• Gather data, complete assessments, formulate treatment plans, and implement treatment.
• Develop and implement aftercare and discharge plans for clients and families.
• Complete required administrative tasks and maintain documentation within deadlines.
