Alignment Health is seeking a compassionate, motivated Bilingual Health Coach & Educator to provide in-home support, wellness coaching, and health education to members throughout Maricopa County, Arizona.

In this highly field-based role, you'll build trusted relationships with members, caregivers, and families while helping individuals adopt healthier habits, access community resources, and improve their overall well-being. Applicants must be comfortable conducting daily in-home visits and traveling throughout Fresno, CA.

What You’ll Do

• Conduct approximately 6-8 in-home visits daily throughout your assigned service area.
• Provide wellness coaching and health education to support behavior change and healthier lifestyles.
• Partner with Advanced Practice Clinicians and Social Workers to develop member action plans.
• Connect members with community resources and social services.
• Support member engagement, retention, and satisfaction.
• Coordinate care with internal clinical teams and external partners.
• Deliver health education materials to members, caregivers, and families.

What to Expect

• Primarily field-based role with daily local travel.
• Monday-Friday, 8:00 AM-5:00 PM PST.
• Visits are scheduled geographically to maximize efficiency.
• Mileage reimbursement provided from door-to-door travel.

Minimum Requirements:

Experience:

• Bilingual Spanish, must be able to write, read, speak
• Minimum 1+ year of experience as a wellness health coach or educator
• Current, valid, unrestricted California Driver's License.
• Reliable personal transportation and ability to travel throughout the assigned territory.
• Ability to maintain automobile insurance in accordance with company requirements.
• Ability to work independently and remotely.
• Strong communication, organizational, time management, and relationship-building skills.
• Proficiency with Microsoft Office (Excel, Word, and PowerPoint).
• Knowledge of health, wellness, nutrition, exercise, and behavior change principles.
• Ability to maintain HIPAA compliance and confidentiality.

Education:

• Associate's degree or two years additional experience in lieu of education.

Preferred Requirements:

• Senior care experience
• Bachelor's degree
• Medical Assistant License

Pay Range: $48,373.00 - $72,559.00 Pay range may be based on a number of factors including market location, education, responsibilities, experience, etc.
