Job summary

Suvida Healthcare seeks a Physical Therapist to join our integrated care team in Phoenix, AZ. Provide patient-centered PT services, develop evidence-based care plans, educate patients, and collaborate with interdisciplinary teams to improve mobility and quality of life for our seniors.

Requirements: DPT or equivalent, AZ PT license, BLS, 3+ years’ experience, geriatric focus preferred, and bilingual (English/Spanish). Proficiency with MS Office is expected.

Qualifications

  • DPT degree or equivalent in Physical Therapy from an accredited program.
  • Active license by the Arizona Board of Physical Therapy Examiners.
  • Current BLS Certification.

Responsibilities

  • Conduct thorough patient evaluations using clinical observation and standardized testing.
  • Develop individualized, evidence-based care plans to improve function.
  • Provide patient-centered education to improve movement and activity.
  • Collaborate with interdisciplinary care teams to set patient-specific goals.
  • Supervise and direct PTAs and PT interns.
  • Lead fall prevention classes and perform fall risk screenings.
  • Integrate Suvida clinical programs into practice workflows.
