Humana is seeking a Care Coach I in Florida to visit Medicaid members at homes or facilities, assess needs, and develop member-specific care plans. Travel is 75–90% locally; you’ll coordinate services and guide families toward resources to promote wellbeing.
The role requires relevant healthcare experience and Florida licenses as applicable, with a focus on coordinated, patient-centered care delivery.
The Care Coach 1 assesses and evaluates member's needs and requirements to achieve and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Care Coach 1 work assignments are often straightforward and of moderate complexity. Reports to the Regional Care Coach Manager.
