Job summary

Catholic Charities Brooklyn and Queens, Inc. seeks a Community Outreach Specialist to coordinate intake, needs assessment, referrals, and service linkage across Brooklyn and Queens within a holistic, family-centered model of care.

Required is a Bachelor’s degree in Social Work or related field, strong communication skills, and willingness to travel to multiple sites, including community events, with possible evenings/weekends.

Qualifications

• Bachelor’s degree in Social Work, Human Services, or related field.
• 2 years of experience working with low income families preferred.
• Proficient in professional social work generalist practice skills.
• Excellent written and verbal communication; interpersonal and transactional skills.
• Bilingual English/Spanish, English/Arabic, English/Cantonese/Mandarin, or English/Russian preferred.

Responsibilities

• Travel to sites throughout Brooklyn and Queens to provide service coordination.
• Provide direct service coordination including intake, needs assessment, and linkage, with follow-up on referrals.
• Work with Office of Mission to respond to emergency community needs.
• Assess eligibility and facilitate distribution of EFSP funds and other grants to assist clients.
• Develop internal and external linkages and promote program services through outreach.
• Document services and referrals accurately in the agency database.
• Track emergency funds assistance and utilization in the database.
• Collaborate with Call Center staff to provide timely access to services.
• Participate in program planning, staff meetings, and group services.
• Assist in development of on/off-site events affiliated with food pantries and parishes.
