Job summary

Family Health Centers of Southwest Florida is seeking a Bilingual Lay Health Educator to support community health education initiatives within our clinic setting. The role includes patient health education on diabetes, metabolic syndrome, and other chronic conditions, conducting individual education sessions, teaching classes using an established curriculum, and maintaining accurate health records.

Applicants should have 2 years of clinical experience as an LPN preferred, be bilingual in English

Qualifications

• 2 years clinical experience as an LPN preferred.
• Previous teaching experience an advantage.
• Bilingual English/Spanish required.

Responsibilities

• Assess patients’ disease self-management behaviors and barriers to change.
• Collaborate with care team to formulate and implement education plans.
• Deliver health education classes using established curriculum.
• Document all education encounters and patient progress.
• Conduct follow-up activities and maintain accurate records.
• Assist with data collection for quarterly reports.
• Perform other duties as assigned.
