CVS Health is seeking a Case Manager Analyst to support optimal member health outcomes by coordinating care, educating members, and facilitating access to services in the Brandon/Riverview, FL area. You will use care management tools and data analysis to assess needs, identify risks, and implement effective care plans with providers and internal teams.

The role requires at least 1 year in behavioral health or long-term care, a Bachelor’s degree or non-licensed master’s level clinician, and

Qualifications

• Bachelor’s Degree or non-licensed Master’s level clinician required.
• 1 year in behavioral health or long-term care setting preferred.
• Bilingual English & Spanish required (verbal and written).
• Proficient with Microsoft Office Suite.

Responsibilities

• Coordinate care and develop effective care plans with providers.
