Responsibilities

• Develops and updates Plan of Care with the interdisciplinary team.
• Provides group, individual, and family therapy sessions.
• Coordinates discharge planning and post-acute services.
• Documents clinical contacts and treatment planning in the EHR.
• Advocates for community resources and ensures safety and compliance.
