Job summary

People’s Arc in Bohemia, NY is seeking a Behavioral Support Assistant to provide enhanced behavioral health supports in our Adult Day Services program. You will implement behavior plans, collect data, and collaborate with BIS and Program Administration to maintain continuity of care.

You will mentor supporting staff, assist with crisis intervention when trained, attend meetings, prepare case action summaries, and help with daily activities and safety.

Responsibilities

• Provide enhanced individualized behavioral health supports.
• Implements behavior plans and data collection as written.
• Meets with BIS and Program Administration regularly to maintain continuity of care.
• Responsible for assisting BIS to ensure data collection is taken correctly and in a timely manner.
• Acts as a mentor to supporting staff and provides ongoing observations and modeling in areas of behavioral health supports.
• As required and with specific training, assists with crisis intervention.
• Assists other Behavior Intervention Specialists as needed.
• Attends meetings with individual and support staff.
• Submits written reports (Case Action Summaries) on significant events which have occurred to the individual in program, the community or residential setting.
• Maintains program setting in a neat, organized manner; adheres to safety standards.
• Brings to the attention of supervisor any changes in the health condition of the individual or any other pertinent information.
• Immediately contacts medical department in case of injury, seizure or changes in individual’s health status.
• As needed, assists individual in/out of van.
• Assists individual, with toileting and personal hygiene.
• Assists individual, as needed, in transferring/lifting.
