Job summary

Community Health Network is seeking a Triage Specialist to support the Crisis Department by coordinating patient care and assessing the severity of mental health concerns. The role involves communicating findings to Crisis Therapists to guide immediate care decisions.

Ideal candidates will have a Bachelor's degree and 1–2 years of experience with behavioral health populations, with a preference for knowledge of local resources and crisis triage experience.

Qualifications

• 4 year/Bachelor’s degree required.
• Knowledge of local resources for behavioral health treatment (Preferred).
• Experience in crisis triage and risk assessment (Preferred).
• 1–2 years of experience with mentally ill or chemically dependent populations (Preferred).

Responsibilities

• Coordinate and care for patients seeking behavioral health services in the Crisis Department.

The essential function is to determine the nature and severity of the mental health problem of patients presenting to the Crisis Department and to give report to the Crisis Therapists regarding these patients.

Exceptional Skills And Qualifications
