Job summary

Olympic Health and Recovery Services (OHRS) seeks a FHARPS Transitions of Care Coordinator to bridge inpatient or correctional releases to community-based support. You will manage referrals, engage participants early, and collaborate with hospitals, housing partners, and community providers to plan seamless transitions.

Qualifications

  • Meets WA state qualifications to be a Mental Health Professional.
  • Master’s degree or further advanced degree in counseling or one of the social sciences; or bachelor’s with five years of direct behavioral health experience.

Responsibilities

  • Screen and track FHARPS referrals and engage individuals early for discharge planning.
  • Collaborate with hospitals, residential facilities, and community providers to develop transition plans.
  • Conduct regular outreach to jails, hospitals, crisis facilities, and other agencies to enroll FHARPS referrals and participants.
