Responsible for providing clinical behavioral health services within a multi-site primary care setting, addressing the mental health, substance use, and behavioral change needs of children, adolescents, and adults. Collaborates closely with multidisciplinary teams across a Community Partner Location, SummitStone Health Partners, and community agencies to support whole-person wellness through brief interventions, behavioral health screenings and assessments, psychotherapy, crisis intervention, care coordination, and case management. Provides consultation and treatment support for innovative services, including Medication-Assisted Treatment (MAT) and chronic pain management, while delivering individual and group therapy. Serves as a resource to staff and providers by assisting with care coordination, problem-solving, and client support needs. Demonstrates a commitment to diversity, equity, and inclusion by fostering an inclusive and equitable environment and delivering culturally responsive services that enhance engagement with and understanding of the communities served.
