Job summary

SummitStone Health Partners seeks a Behavioral Health Therapist to provide clinical services in a multi-site primary care setting, addressing mental health, substance use, and behavioral change needs for children, adolescents, and adults.

The role collaborates with multidisciplinary teams, delivers individual and group therapy, and supports integrated care initiatives, including MAT and chronic pain management, while maintaining documentation and adherence to regulatory standards.

Qualifications

• Master's degree in social work, counseling, psychology, or related field.

Responsibilities

• Provide direct client care, including behavioral health screenings and assessments, individual and group therapy, brief therapy, case management, care coordination, and related clinical services.
• Serve as a clinical resource for building site(s), including medical providers, nursing staff, pharmacy, case managers, and community collaterals.
• Provide/facilitate a wide variety of clinical services including individual, family and group activities to achieve treatment plan goals.
• Collaborate with staff in planning, developing, implementing, delivering, and evaluating clinical services, including coordination with Clinical Managers and Program Supervisors.
• Conduct behavioral health screenings and assessments within the medical clinic setting, including reviewing treatment options and providing referral information.
• Coordinate ongoing care and supportive services across SummitStone Health Partners, Community Partner Location, community agencies, and other external partners to ensure clients receive needed services and support continuity of care.
• Provide crisis intervention services, including crisis assessment, de-escalation, and coordination of referrals to higher levels of care, as needed.
• Partner with medical providers to conduct behavioral health assessments for clients identified as having behavioral health needs.
• Complete clinical documentation in accordance with agency, state, and regulatory requirements.
• Support and foster team values by collaborating with peers and clinic staff to provide assistance and remove barriers to client care.
• Maintain exceptional customer service for internal and external partners by responding to requests and providing information in a timely manner.
• Participate in committees, meetings, and special projects, both internal and external, as assigned.
• Develop and monitor service plans, and treatment progress; document interactions and maintain clinical records and other reporting data for clients/consumers with mild to service mental illness, substance use disorders, and/or disabilities.
• Perform other duties as assigned.

Role Overview:

Responsible for providing clinical behavioral health services within a multi-site primary care setting, addressing the mental health, substance use, and behavioral change needs of children, adolescents, and adults. Collaborates closely with multidisciplinary teams across a Community Partner Location, SummitStone Health Partners, and community agencies to support whole-person wellness through brief interventions, behavioral health screenings and assessments, psychotherapy, crisis intervention, care coordination, and case management. Provides consultation and treatment support for innovative services, including Medication-Assisted Treatment (MAT) and chronic pain management, while delivering individual and group therapy. Serves as a resource to staff and providers by assisting with care coordination, problem-solving, and client support needs. Minimum Qualifications:

• Master's degree in social work, counseling, psychology, or related field from an accredited college or university
• Clinical l icensure (LCSW, LPC, LMFT, CAS Track) or clinical licensure candidacy
• Valid Colorado Driver's license or ability to obtain within 30 days of hire and a clean driving record
• At least one year of directly relevant experience in the behavioral health field
• CPR/First-Aid certification required at time of hire or ability to obtain within 30 days of hire
• Must not be listed on the Office of Inspector General (OIG) List of Excluded Individuals and Entities (LEIE) or otherwise be excluded from participation in federally funded healthcare programs.
• Must maintain all required health screenings, immunizations, vaccinations, and/or titers in accordance with organizational policy, regulatory requirements, and contractual obligations. Required immunizations may include, but are not limited to, Influenza, MMR, Varicella, Tdap, and Hepatitis B.
• COVID-19 vaccination is encouraged but not required unless otherwise mandated by applicable law, regulation, client contract, or organizational policy.
• Must comply with all applicable credentialing, verification, and screening requirements, including SAMHSA verification standards when applicable to the position.

Preferred Qualifications:

• Experience with mild to severe mental illness and substance use disorders
• Previous experience in an Integrated Care or Primary Care setting (or medical systems)
