Community Health Network seeks a Behavioral Health Therapist Acute to provide direct patient care, assessments, and treatment planning. The role emphasizes collaboration, discharge planning, and comprehensive referral services to ensure holistic patient care.

You will conduct psychosocial assessments, develop individualized plans, and deliver evidence-based interventions, including crisis management, while coordinating with a multidisciplinary team for optimal outcomes.

Qualifications

  • Master’s degree in mental health field required.
  • 2+ years of experience providing mental health treatment required.
  • License eligible for the LCSW, LMHC, LMFT, or LCAC in Indiana.
  • Commitment to customer satisfaction for internal and external customers.
  • Provides comprehensive psychosocial assessments.
  • Develops individualized treatment plans with focus on home/school/work/community functioning.
  • Provides various modalities including individual, family, and group therapies and crisis intervention.
  • Discharges planning and coordination of care with appropriate resources.
  • Ensures efficient use of material and human resources for quality care.
  • Collaborates with a multi-disciplinary team in assessment, referrals, and discharge planning.
  • Active involvement in professional development and service line quality improvement.
  • Adheres to Behavioral Care Services policies and participates in Aggressive Behavior Management Training.

The Behavioral Health Therapist Acute provides direct patient care through bio-psychosocial assessment, treatment plan development, conjoint/family therapy sessions focusing on safety and discharge planning; case management, collateral consultation and comprehensive referral services.

Exceptional Skills and Qualifications

Applicants for this position should be able to collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving.

  • License eligible for the LCSW, LMHC, LMFT, or LCAC in the State of Indiana required.
  • Master’s degree from an accredited college or university in a mental health related field required.
  • 2 + years of experience providing mental health treatment required.
  • Recognizes and commits to critical importance of customer satisfaction (both internal and external customers).
  • Provides comprehensive psychosocial assessments.
  • Develops individualized treatment plans with a focus on improving individual and family functioning in the home, school, workplace or community. Provides effective and efficient mental health interventions including, but not limited to individual, family and process group therapies and crisis intervention appropriate to client needs.
  • Provides comprehensive and fluid links to other treatment programs within the continuum as is appropriate to patient treatment needs. Documentation and communication regarding all plans is clear and timely.
  • Develops and implements effective and efficient treatment discharge plans as is appropriate to client needs.
  • Ensures appropriate utilization of material as well as human resources required to provide quality and cost-effective care.
  • Collaborates with multi-disciplinary treatment team in planning, facilitating, and coordinating treatment especially in the areas of assessment, referral resources, and discharge planning as is appropriate to the patient treatment needs.
  • Actively involved in professional development and education.
  • Actively involved in service line quality improvement efforts.
  • Complies with all applicable Behavioral Care Services policies and procedures. Participate in Aggressive Behavior Management appropriate to the population served. See Behavioral Care Services Policy: Provision of Aggressive Behavior Management Training to Staff.
