Job summary

Centers for Independence in Milwaukee, WI is seeking a Direct Care Worker for adults with severe and persistent mental illness. This full-time role provides a homelike environment and supports residents in daily living skills, meals, hygiene, transportation, and community participation.

The position requires a High School Diploma or GED, at least 1 year of relevant experience, and completion of CBRF training with competency testing. Strong communication and teamwork are essential.

Qualifications

• High School Diploma or GED is required.
• Minimum 1 year experience working with adults with severe and persistent mental illness.
• Attends required annual CBRF training and passes competency testing.

Responsibilities

• Assists consumers in learning independent living skills such as laundry and cooking.
• Prepares and serves nutritious meals as directed; trains residents in food preparation and serving.
• Monitors medication adherence as directed by physician.
• Reports to Master’s Level Clinician, WHCG RN, or physician any changes in client behavior.
• Ensures residents maintain acceptable standards of personal hygiene.
• Facilitates transportation to day programs, jobs, or school as needed.
• Keeps house clean and complies with DHS 83 CBRF regulations.
• Reports any repairs to the Property Management Department.
• Documents residents' daily behavior in the daily log per ISP goals.
• Attends staff meetings, in-service trainings, and conferences.
• Adheres to established policies and DHS 83 rules.
