SCAN Group in Southern California is seeking a Behavioral Health Specialist, Embrace to collaboratively assess and plan members' mental health needs within a collaborative care model.
You will provide short-term treatment, manage cases, and coordinate with care teams while following HIPAA and quality guidelines. Licensure in LCSW/LMFT and 2–5 years of behavioral health experience are preferred.

The purpose of the Behavioral Health Specialist, Embrace is to collaboratively assess, plan, implement, coordinate, monitor, and evaluate the member's mental health needs through a collaborative care model. Focusing on members' behavioral and mental health serves to wholistically support the member and potentially prevent unwanted or unnecessary events across the continuum of care.
