Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to evaluate and treat home-bound patients in Weatherford, TX. You will coordinate with an interdisciplinary team, develop care plans, and perform nursing interventions within scope of practice.

The ideal candidate holds an RN license with 1–2 years of psychiatric/home health experience, a valid driver’s license, CPR and Hepatitis certification/declination, and reliable transportation.

Qualifications

  • Must be a graduate of an accredited nursing program (Diploma/ASN/BSN).
  • 1-2 years nursing experience, preferably in acute psychiatric/home health.
  • Current RN state license.
  • Driver's license and reliable transportation.
  • CPR and Hepatitis consent/declination.
  • Ability to work in home health and interdisciplinary teams.

Responsibilities

  • Conduct in-home psychiatric assessments and psychosocial evaluations.
  • Develop, implement, and update individualized plans of care.
  • Provide skilled nursing interventions for behavioral health and related medical conditions.
  • Coordinate care with physicians, families, and referral agencies.
  • Ensure patient medication profiles are current and accurate at all times.
  • Complete admissions, resumptions of care, recertifications, and discharges per standards.
  • Demonstrate knowledge of OASIS and ensure proper documentation.
  • Maintain EMR documentation and timely visits; meet productivity expectations.
  • Participate in QA, audits, and on-call rotations; adhere to HIPAA and standards.

Coordinates care with the interdisciplinary team, patient/family, and referring agency; the BHRN also assumes the responsibility for coordination of care.Essential Job Functions:Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial, and environmental needsDevelop, implement, and regularly update individualized plans of care to ensure quality and continuity of servicesProvide skilled nursing interventions related to behavioral health and applicable medical conditions within scope of practiceCoordinate care with physicians, interdisciplinary team members, patients, families, and referral agenciesEnsure patient medication profiles are accurate and current at all timesComplete admissions, resumptions of care, recertifications, and discharges in accordance with regulatory standardsDemonstrate working knowledge of OASIS and ensure documentation supports medical necessity and homebound statusComplete visit documentation accurately and within required timeframes, including same-day SNVs and OASIS within 48 hoursUtilize appropriate behavioral health and medical assessment screening tools and evidence-based practices to evaluate and support patient needs.Monitor assigned cases to ensure compliance with payer, federal, and state requirementsSupervise and provide clinical direction to Home Health Aides as applicableParticipate in quality assurance activities, including responding to QA and coder reviews and performing clinical record auditsMaintain proficiency in EMR systems, including timely syncing of visits and device maintenanceMeet established productivity standards, including visit volume, meetings, and assigned dutiesParticipate in mandatory meetings, case conferences, and on-call rotation, including weekends and holidaysAdhere to organizational policies, HIPAA requirements, and professional standards of conductDemonstrate sound clinical judgment, accountability, adaptability, and effective communication under routine and stressful conditionsQualifications:Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.Must have 1-2 years of nursing experience (within the last 3 years preferred) in an acute treatment unit in a psychiatric hospital, psychiatric home care, psychiatric hospitalization program, or other outpatient psychiatric service.Current state license as a Registered Nurse.Current state Driver’s License.Proof of current CPR and Hepatitis consent/declination.Reliable transportation with valid and current auto liability insurance.We are an equal opportunity employer.
