CHAS Health's comprehensive compensation package\* includes:* **Annual milestone bonus** for all eligible employees! $500 for every year worked, up to $5,000.* **Robust health coverage** with employer-offered medical, dental, vision, long-term care, and life insurance.* **401(k) with up to 6% employer match** for a retirement plan that invests in your future.* **Work/life balance**—enjoy more than six (6) weeks of paid time off (PTO) annually for full-time employees, to recharge and refresh.* **Tuition assistance and student loan repayment**—let CHAS Health pay for your education!* **Up to $7,500 for childcare** annually through reimbursement, to help you balance work and family life.* **CME and license fee reimbursement**—we've got you covered. ***\***Offered benefits dependent upon employee eligibility and criteria***Job Description:****Purpose of Job:** Improve the overall health of the communities we serve by providing social or behavioral health services to patients and families by performing the following duties:**Essential Duties and Responsibilities:*** Provides individual and group counseling services within individual scope of practice to assist in clients in achieving more effective personal, social, educational and vocational development and adjustment.* Available to primary care providers for behavioral health staffing and consultation.* Collects psycho-social data about individuals and provide gathered information to providers to assist them in managing medical care.* Perform mental health evaluations utilizing approved evaluation tools assessing amenability to mental health treatment and level of care needed.* Develops detailed care plans for each client and document as part of the electronic health record.* Links clients with various community resources.* Provides in-service training for CHAS staff as requested.* Participates in community outreach activities as directed.For staff with chemical dependency expertise:* Responsible for performing chemical dependency assessments on clients using approved testing instruments and evaluating tools and determining their amenability for referral to other agencies and/or programs.* Conducts individual and group counseling and education focusing on alcohol and drug treatment, relapse prevention, and cognitive behavioral change.For staff with clinical supervision training and duties:* Supervises and coordinates volunteers and behavioral health / social work students as required.* Facilitates peer review and chart audit process as assigned.* Performs other duties assigned, including supporting the CHAS Health Mission and Core Values.**Qualifications:**Education/Experience: Valid state licensure as a Licensed Advanced Social Worker (or associate), Licensed Independent Clinical Social Worker (or associate), Licensed Mental Health Counselor (or associate), Licensed Marriage and Family Therapist (or associate), Licensed Clinical Professional Counselor, and/or Licensed Professional Counselor required. Two years of clinical practice with underserved populations desired.Skills: Knowledge of available community linkage resources. Excellent oral and written communications skills required.Computer skills required. Work requires consistent exercise of independent judgment and discretion. BLS (CPR/AED)required. Valid drivers’ license and insurance required. Commitment to supporting a safe and respectful environment is required.**Physical Demands:**Must talk, hear, and sit more than two-thirds of the day. Using hands occurs over two-thirds of the day, while reaching with arms occurs less than one-third of the day. Communicating is required over two-thirds of the day, while climbing, stooping, or crawling is minimal.
