Job summary

Family Health Center of Southern Oklahoma seeks a Behavioral Health Provider (LPC, LCSW) to deliver direct behavioral health services under supervision while promoting comprehensive patient care.

The role requires current licensure, ability to perform evaluations, complete required documentation, coordinate referrals, and participate in PCMH initiatives to support quality outcomes for diverse patients served by FHCSO.

• Eligible for malpractice insurance.
• Maintains CME requirements and credentialing criteria.
• Basic computer skills needed.
• Physical and mental capability to perform duties.

Responsibilities

• Provides direct behavioral health services to patients under supervision.
• Conducts initial and ongoing behavioral health evaluations.
• Completes required paperwork per policy and regulation.
• Develops and monitors patient management plans with progress notes.
• Counsels patients on treatment regimens and daily living challenges.
• Documents encounters clearly on the day of service.
• Maintains licensure and credentials current.
• Initiates referrals and communicates with clinicians or agencies.
• Supports Health Disparity Collaborative activities and data entry.
• Attends provider meetings to maintain skills.
• Consults with medical physicians when appropriate.
• Participates in community resources for continuity of care.
• Engages in PCMH projects as assigned by the Chief Medical Officer.
• Follows FHCSO mission and vision.
• Performs any other duties as assigned.

JOB SUMMARY:

As an integral member of the provider team, provides direct behavioral health services to patients under direction and responsibility of a licensed psychologist/licensed psychiatrist/licensed clinical social worker.

RESPONSIBILITIES:

• 1)Provides direct patient care, performs comprehensive examination, obtains a thorough and appropriate behavioral health history, compiles patient data and arranges testing if needed, therapy, and follow-up for behavioral health patients of all ages.
• 2)Be responsible for initial behavioral health evaluations.
• 3)Complete paperwork as required by statute, regulation and/or FHCSO Policy and Procedures.
• 4)Develops and implements patient management plans, records progress notes, and assists in provision of continuity of care for behavioral health patients.
• 5)Instructs and counsels patients regarding compliance with prescribed therapeutic regimens, emotional problems of daily living.
• 6)Completely documents each patient’s encounter clearly and concisely, on the day of service.
• 7)Maintains up to date required licensure and credentials.
• 8)Initiates patient referrals as necessary, and communicates with the referral clinician or agency.
• 9)Support activities related to the Health Disparity Collaborative. Duties include but are not limited to supporting the incorporation of staff time for data entry, team meetings, production and submission of monthly reports, maintenance of database.
• 10)Maintains and improves skills by attending health center provider meetings.
• 11)Consults medical physician when appropriate.
• 12)Participates in community agencies and/or resources in providing continuity of care.
• 13)Participates in designated activities and projects for PCMH as assigned by the Chief Medical Officer.
• 14)Follows the mission and vision statement.
• 15)All other duties as assigned.
