Bozeman Health is hiring a Psychiatric Services Patient Navigator to assist patients with complex psychosocial needs and eligibility for social programs. You will help develop discharge plans for high-risk populations and receive referrals from an interdisciplinary care team.

The role requires a Bachelor’s degree in a related field, AHA BLS certification, and at least 6 months of community resource experience, with growth opportunities in a supportive health system.

Qualifications

  • Six months of Community Resource experience.

Responsibilities

  • Collaborates with crisis/behavioral health staff to develop a person-centered plan.
  • Identify social determinants of health needs and connect to community resources.
  • Track high-frequency/at-risk patients and update the care team.
  • Advocate for patients and assist with patient assistance programs.

Job description

The Psychiatric Services Patient Navigator assists with patients who have complex psychosocial needs and require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. Participates in the development of a discharge plan of care for high-risk patient populations. Receives referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, Case Managers, staff nurses and other members of the care team). Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patient served. Must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles. All age groups are served by this position.

Required
Minimum Qualifications
  • Bachelor’s Degree in Community Health, Medical Social Work, or related field
  • Current American Heart Association (AHA) Basic Life Support (BLS) certification.
  • Six (6) months of Community Resource experience
Preferred
  • Master’s of Social Work (non-licensed)
  • One year community resource management experience
Essential Job Functions

In addition to the essential functions of the job listed below, employees must maintain timely completion of all required education, trainings, and certifications as assigned and in accordance with Bozeman Health policies, department-specific requirements, applicable certification program and registry requirements, and other applicable regulatory requirements.

  • Collaborates with the Crisis Intervention Specialist or Behavioral Health Specialist to develop and implement an individualized person-centered treatment plan.
  • Identify needs in social determinants of health and connect the patient and family to community resources
  • Track high frequency/at risk patients throughout the course of care and update team of potential issues
  • Advocating for patients and their needs
  • Helps with patient assistance programs
  • Assists and set ups Telehealth support for patients
  • Other related duties and responsibilities as assigned.

Exposures (Continuously): Bloodborne pathogens, such as blood, bodily fluids, or tissues. Radiation in settings where medical imaging procedures are performed. Various chemicals and medications are used in healthcare settings. Job tasks may involve handling cleaning products, disinfectants, and other substances. Infectious diseases are caused by contact with patients in areas that may have contagious illnesses. Emotionally challenging situations, such as dealing with distressed patients or difficult family interactions.
