Qualifications

• Safety-Care Specialist within 60 Days required.

Responsibilities

• Provides brief interventions and psychoeducation to support patient wellness and recovery.
• Works in a highly collaborative manner with therapy clients' assigned case managers and other service providers, within and outside facility.
• Assists in developing measurable individualized therapy goals and objectives.
• Serves as the primary contact for all payors regarding utilization review and management issues.

A Brief Overview

Serves as a contributing member on the Behavioral Health team; provides screening and assists with patient disposition plans that present with a health crisis. Helps Behavioral Health patients connect with resources that allow them to manage mental crisis as they arise; improve overall health and potentially avoid hospital encounters. Assists with posting discharge care and connecting patients to community resources and supports for individuals with complex circumstances.

Essential Functions

• Provides brief interventions and psychoeducation to support patient wellness and recovery.
• Works in a highly collaboratively manner with therapy clients' assigned case managers, and other service providers, within and outside facility.
• Assists in developing measurable individualized therapy goals and objectives.
• Serves as the primary contact for all payors regarding utilization review and management issues.
• Delivers support of clinical integration needs of other clinical services, and collaboration with medical director leadership.
• Maintains a patient first philosophy and engages in service recovery when necessary.
• Participates in clinical huddles when working with medical & behavioral health providers to review the appointment schedule for the day and determine which patients will need visits.
• Ensures that documentation compliance requirements are met consistently.

Education Qualifications

• Bachelor's Degree from an accredited college or university in Social Work or related field required

Experience Qualifications

• Experience working in a clinic or hospital setting preferred

Licenses and Certifications

• Basic Life Support through the American Heart Association upon hire required
• Safety-Care Specialist within 60 Days required
