Responsibilities

• Perform initial, ongoing and functional health status assessment and document data from patients, significant others, and communities.
• Develop and coordinate care plans and medical management plans; promote cost-effective care and smooth care transitions.
• Evaluate response to nursing interventions and revise plans of care as needed within a multidisciplinary team.
• Delegate tasks and supervise staff, ensuring continuity of care and collaboration with the health care team.
• Participate in process improvement activities to achieve quality, safety and patient satisfaction targets.
